After previously speaking out about the criticism he faced regarding his appearance, Barry Keoghan confirmed he had undergone a cosmetic procedure to tighten his skin.
Keoghan, whose credits include “Eternals” and “Saltburn,” can be seen in a video shared on Instagram by skin and beauty expert Nilam Holmes, founder of the Dermaspa Clinic, last week. It shows him receiving XERF treatment, a non-invasive procedure aimed at improving “mild to moderate skin laxity” on his face.
“I feel better about myself,” the Irish actor says in voiceover. “I look more beautiful and I think it’s important to take care of yourself inside and out.”
Elsewhere in the clip, he acknowledges that this wasn’t his first visit to Dermaspa: “Every time I come here, it’s very relaxing and therapeutic.” »
In his caption, Holmes noted that this was Keoghan’s second XERF treatment.
“I’ve been working with Barry for a while now, taking care of his skin through different films, campaigns and projects and the trust he puts in me means a lot,” she explained. “His skin has really improved over the last few years and he looks great.”
Keoghan, she added, “offered to let me film him doing it.”
According to the Dermaspa website, XERF “is designed to be comfortable” and unlike other procedures, does not require the use of needles or numbing cream. While clients often “see a noticeable result from just one session,” most are encouraged to receive two treatments “to build and extend the initial result.”

A single facial and neck session costs around $3,300, and two treatments cost around $6,000 per Dermaspa.
The clip, however, received a surprisingly divided response from some of Keoghan’s fans.
“I always thought you looked good in your movies. You didn’t really need to do anything,” one person wrote. “Don’t listen to those people in Hollywood, they make you feel like you have to be perfect, but if it makes you feel good, there’s no harm in doing a little treatment to make you feel more confident, but you look good.”
Another added: “Don’t let people’s harsh words affect you. Honestly, I think you’re pretty hot!”
Others, however, praised Keoghan for his “brilliance”, as well as his frankness about it.
“Her skin is absolutely flawless and brand new,” one person wrote, while another noted, “I will definitely get this treatment when I win the lottery.”
Nominated for an Oscar for his performance in 2022’s “The Banshees of Inisherin,” Keoghan has been candid about the criticism — some of which is heavily focused on his appearance — he’s received over the years.
“There’s a lot of hate online,” he said in an interview with SiriusXM in March. “There’s a lot of abuse about my appearance, and it’s a little past the point of… you know, everyone experiences this… but it made me run away. It made me really go into myself, not wanting to go places, not wanting to go out.”
About a month later, Keoghan said he had “stopped going to events” amid rumors he cheated on his ex-girlfriend, pop star Sabrina Carpenter, before their split in 2024.
“I stopped just socializing, and again, it was because there was a narrative that was never really brought up,” he explained in an episode of the “Friends Keep Secrets” video podcast. “A story that is not true and I never confirmed or said anything about it, and I just disappeared.”
Keoghan, 34, is currently working on a series of projects that look set to propel his Hollywood profile to new heights in the years to come. He is expected to play the Joker opposite Robert Pattinson’s Batman in “The Batman: Part II”, which is scheduled for release in 2028.
He was also seen in a Ringo Starr costume for Sam Mendes’ highly anticipated four-part film, “Beatles,” which will also star Harrison Dickinson, Paul Mescal and Joseph Quinn as John Lennon, Paul McCartney and George Harrison, respectively.
